Form can only be completed by a Volunteer On-Ice Helper that is a registered North London Nationals player and on a North London Nationals Official Roster for 2025-26 Season.

Required Equipment Policy

Registered North players must ALWAYS be in full hockey equipment if they are helping another team.

On-Ice Helper Policy

All ice helpers must be at least 12 years of age and at least two years older than the team they are assisting.

Injured players are not allowed to participate as an on-ice helper until they are cleared to return to play for their team.
