Covid-19 Vaccine Validation
Submitted By Andrea Maillet on Tuesday, September 14, 2021
Alliance Hockey and GLHA recognize the significant impact that the COVID-19 pandemic has had on sports and recreation. We have heard how important hockey is to our families, coaches and participants and are committed to doing what is necessary to keep our rinks as safe as possible for the coming season.

In keeping with the Middlesex-London Health Unit’s most recent recommendations for amateur sports leagues, the Alliance Hockey and GLHA will require proof of COVID-19 immunization for all coaches, volunteers, bench staff and participants who are eligible for vaccination (including those born in or before 2009), unless exempted. Eligible participants require proof of having received their first dose before their first skate and be fully vaccinated by October 31, 2021.
As our community begins to see the effects of Wave 4 of this pandemic and as the frequency of COVID-19 cases in amateur group sports continues to increase, we believe mandatory vaccination is an important step to support our community’s efforts to prevent the spread of this virus. Ensuring that all eligible individuals are vaccinated will provide us the best opportunity to keep our participants on the ice and will support larger cohorts and regular team play. Further details regarding implementation of this policy will follow in the coming weeks. Please note all vaccination will be kept confidential by our Board of Directors. Any individual not complying with these regulations will be removed from the ice as of November 1st.

You can submit your vaccination information here.

How to access your vaccine receipt
After receiving a dose of COVID-19 vaccine, a receipt will be sent via email. If you did not receive an email confirming your first dose or second dose of vaccine, please caccess your vaccination receipt through the provincial COVID-19 vaccination portal.
NOTE: you will need your date of birth, postal code and health card information in order to access your vaccination receipt online. You can only access your vaccination receipt through the provincial COVID-19 vaccination portal if you have a green Ontario Health Card and presented the card at the time of your vaccination.
